This is Philips 570408, from the Philips MainsFit range, a T8 / T12 lamp. Philips prints the order code on the sleeve and on the lamp itself, and it is the surest way to repeat an order - ask for 570408 and you get the same lamp, whatever the packaging says that season.
What Philips MainsFit means
Philips MainsFit LED tubes are Type B: they wire direct to line voltage and the fluorescent ballast is removed. That eliminates ballast failures and ballast losses for good, but the rewiring is an electrician job - it is not a swap.
T8 and T12 linear tubes: what T8 / T12 means
Linear tubes on a medium bi-pin (G13) base. The length and the wiring type are the two things to match: a 4 ft tube is 48 inches, and an LED replacement is Type A (runs on the ballast), Type B (direct wire, ballast removed) or Type A+B (either). The wiring type decides whether an electrician is needed.
